Preparing for Preventative Care Visit

I’ll bite my tongue on the ridiculousness of the system. But seeing as a preventative care appointment is fully covered by my insurance but not a “medical treatment” appointment, I’m looking for suggestions on how to prepare for the visit and thus avoid a surprise medical bill. In particular, how to answer if the doctor asks open ended questions like “do you any questions or concerns” or when they go through the routine questions about my mental, sexual health etc. Am I meant to not answer? Or answer vaguely? Or hint at something and hope they talk about it? Would a list of what is recommended for a female of my age help me avoid potential landmines? (Additional side question: I routinely get strong headaches, for the past 20 years or so, it’s not anything major (hopefully?) but would be nice to prevent them from continuing to happen if possible. Everything I’ve read makes it sound like I shouldn’t mention this at all, and instead make a separate appointment just for that?). Thanks!
