Provider accepted insurance and now is billing me because insurance is now ineligible. Help -CA

I went to the doctor for a normal appointment $25 copay with my insurance. She did bloodwork and I came back for a follow up the next week. A month has gone by and I go to pick up my monthly prescriptions. They tell me I can’t pick up prescriptions because my insurance didn’t cover my follow up appointment and I owe $170. A regular appointment is only $120 but I need to pay what they charged the insurance company. Nothing extra was done at this appointment just a standard visit. I looked at the bills and it seems like the follow up appointment wasn’t submitted by them until a month later. My first appointment was covered and submitted right away.

I may be totally at fault but something seems very wrong. Why would I have to pay $170 instead of $120? Why would they tell me my insurance was covering the appointment and then a month later tell me it wasn’t.
