Q: Telling the truth on state taxes

So my question is thus: I believe that I may have had some coverage last year in Massachusetts, but only a few months, if even. I have no idea how to check. Either way, my refund changes from $200 to -$1300 if I claim that I had zero healthcare over the course of the year.
What are the penalties if I don’t tell the truth here? To be fair, I’m not sure HOW to tell the truth here, there’s a good chance I had no insurance at all. If the worse case scenario is that they fix my “error”, then it’s worth it, right? Or perhaps I am better applying for an extension to take the time to figure it out? (Tax day is MA is the 19th, due to The Patriots’ day {go Pats!})

It just kinda puts a damper on my 3000 dollar federal refund, which I need desperately- I make about 8X as much money in the summer as I do in the winter as a server. I’ve never had to owe money before, so I don’t even know how that works. Does it come out of my other refund?

My gut says to apply for state extension and file when I have more money. Right now I need my other refund in full and could care less about scoring 200 bucks.
