I just got a health statement on behalf of my mom who passed approximately a year ago. Suddenly several charges from Feb of 2020 are coming back saying that service was denied/”these services require prior approval of a plan of treatment, (..) treatment forms are required”. My mom has been dead a year. I am her trustee and we didn’t go to probate so I don’t have the forms the insurance company wants in order to interact with them directly so I have no idea how to appeal this. She apparently now owes $22k. Please help.
