In my Benefits Summary, there’s a Specialist Co-Pay of $50 and there’s another line for Physical Therapy (80% after deductible). I asked my insurer if I need physical therapy, do I pay $50 or 80% after deductible. They said it depends on how the therapist code the visit. What does this mean?
