Sudden charge for anesthesia after colonoscopy

I had a colonoscopy last year with my in-network doctor and was told it would be completely covered by insurance.

This year, I started getting bills for almost $500 for an anesthesiologist. When I call insurance, they say they’ve requested the doctors notes and some recommendation that this was medically necessary for them to cover anesthesia instead of just twilight. Unfortunately, they haven’t received what they need.

When I call the billing office, they tell me “yeahhhh…your insurance is very particular. They usually don’t cover this.” When I ask why they haven’t sent in the requested forms, they say “well, we can try but it’s very difficult for them to cover it.” I tell them that I was told the procedure would be completely covered which, of course, I do not have in writing. When I ask why anesthesia was used if it’s not medically necessary, they say “that’s just how we do things here.”

I’m at a loss. Am I out the $500? This has been going on for months. I don’t want it to screw up my credit.
