Switched jobs, need help finding insurance especially for a national plan

I work remote and I split my time between 2 states. I live in NYC however all my doctors are in SC and I often fly home to see and want to keep them there.

I have 1 cardiologist in SC. I see him 1-2x a year.

I would like insurance to cover SC and NY. NY if I need to see a doctor or have an emergency and SC to keep my old doctors.

I am OK with getting catastrophic health insurance if it makes sense.

I take 1 prescription drug. Costs about $10/mo. Not an issue. This may increase to 2 prescription drugs but nothing crazy expensive.

I would also like to see the PT for dry needling or chiropractor. Insurance doesn’t usually cover things but throwing this out there if something makes sense.

I often have panic attacks every two years, so I would like decent ER coverage. I never plan on having these and going to the ER but it happens may times. I did get my heart checked out with a nuclear stress test 2 years ago for the chest pains and everything was perfect.

Do I get health insurance in SC or NY state?
