Unwanted Pregnancy and Ultrasound Testing

I was hoping to get some advice regarding a pregnancy test that I didn’t ask for. I went to get a urine analysis for chlamydia and gonorrhea. I honestly didn’t even want that test as I’m in a long term relationship, but my doctor insisted to rule out any cause for my symptoms. In that urine analysis, she tacked on a pregnancy test. I had an ultrasound immediately following the lab tests, so if I were pregnant they would have seen it then. The ultrasound was both pelvic and transvaginal, which they just did without any explanation. The transvaginal was not covered by my insurance either.

Is this standard or normal? I tried to appeal but they wouldn’t listen to me. The bill for the pregnancy test was over $50, at which point I’ll just take a store bought test. The ultrasound was over $250 (after I had already hit my deductible).

Edit: I live in Massachusetts and have Aetna.
