What happens when a claim is denied?

I’ve had a few diagnostic claims denied but the EOB always shows I’m responsible for $0 of the original claim. Whats happening when claims are denied like this? Will I be responsible for the full claim amount or is the EOB accurate when it shows a claim is denied but I’m responsible for 0? Located in MI, insurance through Cigna
