Hi, I’m trying to fill out a claim form for BCBS (I’m in CA) and I’m confused about what to write. I’m doing it online and it’s the first page, where they ask date and reason for visit etc. It says “provide details about the injury, symptoms, and treatment received”. I went in for a follow up visit with my rheumatologist to continue my medications and just recheck.
