Who pays when there are no in-network surgeons?

My father has a marketplace plan in Texas, has met his out-of-pocket max for the year, and needs oral surgery. He’s talked to his insurance carrier, who confirmed that there are no in-network surgeons in his area. He believes this means he’ll be on the hook for the full cost of the surgery. Is this correct? He has a consult on Wed because he must get this done asap (some kind of infection), but I’m wondering if I can help minimize his out-of-pocket expense. Thanks.
