Why did Aetna assign me a Care Manager?

I’m 21F in California.

A couple months ago Aetna assigned me a Care Manager who is also a Registered Nurse. Every month she calls me and just asks me how I’m doing. Like asks for updates on all my medical, if I have any concerns, etc. I will admit that I do see a LOT of doctors. I’m not going to die or anything, but I do have a lot of medical issues like sleep apnea, history of mental illness, chronic pain, etc.

She’s very nice and all, but I’m interested as to why Aetna assigned me a Care Manager? It would be one thing if was advising me to do this test or see this specialist, but all she really does is a check-in. Like what does Aetna or I gain from this?
