There is a form I need to fill out where I need to put down my ins. information. I’m pretty sure the procedure I want is not covered by ins. and I plan on paying fully out of pocket anyway. Will my insurance company be automatically notified by the clinic (dermatology)? Their website says “You will be given a standard insurance report which is acceptable to all insurance companies. Please attach this form to your completed insurance form and submit directly to your insurance company.”
