I recently got a bill for a doctors visit back in August. Three charges on the bill (totaling about $1k) did not seem to go through my insurance. I’ve combed through all the EOBs from that visit and find nothing that matches those charges. Am I right in thinking that the doctor is required to submit all charges to the insurance before billing them to me?
