I chose to not have my wife as a dependant during an enrollment period. I THOUGHT we were getting a divorce. We are separated, but I don’t think it will be a divorce. Now I’d like to have her covered.
I have to provide documentation, which is none really. The description of claim however, I can go all day justifying why she needs coverage. Mental issues, WOMEN issues, prescriptions. I just hope someone out there has been through this. The “Claim Initiation Form,” asks for a detailed description so it can be reviewed by a “Benefit Determination Review.” Any advice is appreciated.
