Insurance denied his recent X-ray from an in network podiatrist. What?!

My boyfriend has a lot of pain in his ankle from injuring it a couple years ago. He finally went to a podiatrist about it 3 weeks ago. Her response seemed a little too good to be true, so he got a second opinion somewhere else today. At both locations, they took xrays as that is standard practice. He just got a robot phone call from his health insurance saying his last imaging was denied and he will be mailed a letter explaining why and how to appeal if he thinks it’s an error. Both doctors were in network. What the heck? Why pay for insurance if they pull this bs? Has anyone experienced this or have an idea of why this happened? It’s nerve wracking because we don’t have the money right now to pay for this unexpected bill that absolutely should be covered by insurance. His insurance is United PPO which he gets through an employer.
