Provider overbilled insurance: normal?

I paid a flat fee of $300 for a service. Paid at time of service.

I have a high deductible PPO plan. I see the claim they sent my insurance is for $700 and that I “may owe” $600. I called the provider and they said the bill was settled with my $300.

I guess I’m just confused. It kind of seems like it’s fraud and they are trying to overbill the insurance carrier.
