Service rendered vs. Billing Cycle

In late October I went in for a blood draw and panel for a health issue I was having. I had hit my OOP deductible for 2020 back in June. I assumed it would be fully paid and covered by my health insurance.

Today I get an over $500 bill for the service rendered (showing that I am 100% responsible for the entire bill) and the bill date was Jan 1 – effectively meaning that now I must pay this entirely out of pocket as I have obviously not met 2021 deductible yet.

How do I fight this? Who do I call? Quest Diagnostics? My doctor? My insurance? I am at a loss as to how to even go about this, but this seems totally wrong.
