Short term insurance for a US citizen who has never lived in the US – HELP PLEASE :)

Hi! I have LOTS of questions about how does the USA health insurance situation works. First, let me tell you about my situation:

I am 22 and a US citizen, but I moved abroad when I was little and I have never been back. Now, I am planning on living in NYC for about 3 – 5 months. I currently live in Spain where healthcare es free for all and I have never paid any doctors appointment or basically anything.

I was told I have to get an health insurance, but I don’t know exactly how it works. Is it compulsory to have an Insurance? What happens If I don’t get one? Can I get one for only thee months?

How much should it cover? Do insurance’s cover health problems in the whole USA or do I have to get one specifically for NY? Do I have/get any benefits from being a US citizen?

Finally, I was told that even if I get an insurance I have to pay for a doctors appointment. Is this correct?
