Should past provider cover hardware removal surgery?

Hi. I thought this would be an easy google but it wasn’t. I had a hip broken and 2 surgeries done with plate inserted. 2 years later it needs to be taken out(am 24 yo). I’m trying to choose new insurance for 2021 and it seems like it’s going to cost massively to have this done.

I need to know if the insurance company that covered the initial injury should be or might be responsible for costs of removing the hardware. i haven’t been on said company for a year now. Also avoiding calling asking said company because they will most likely lie and claim non responsibility knowing them. Please help

If they are that would make life a lot easier.

Other info: trying not to make my unique identity obvious but I’m living international in New Zealand I’m American from California And my income is pretty much nothing due to job issues. Parents will help pay for plan.

And if somebody also has suggestions about insurance plans for said New Zealand immigrant those are welcome.
