TX-Help me choose best employer-sponsored plan (long, sorry!)

Back story—spouse lost job early 2020, & recently re-employed after 10 mos (but not yet at former pay rate). Former employer’s plan was PPO, 80/20, Medical (UHC), Dental (Ameritas), Vision (VSP) with $2000 IN/$4000 ON deductibles. $2347/mth COBRA. We stayed on it thanks to ARP/CARES provision April-September, then continued since deduct was satisfied and daughter was facing a possible $$$ surgery (has been delayed)

DH is 60yo, high cholesterol & sleep apnea but otherwise healthy. Family history of stroke/heart issues

I’m 55yo, thyroid disease, fibromyalgia but otherwise good health. Family history of heart issues, cancer & diabetes

3 kiddos on plan (20yo, 18yo both college students, 12yo athlete). 18yo would like option to elect for surgery this year, but surgeon is in another state so not sure new plans will allow for us to seek care outside our current state? Currently both students live with us but 18yo will be in NY in the fall.

My question: we have 4 plan options available to us. None are the familiar 80/20 with IN/ON deductible we are used to. It’s tempting to take the least expensive premium coverage since we really want to rebuild our drained emergency fund. But we want to make the best decisions for all care needs

Anthem Premium HDHP w/HSA $1500/$3000 IN & $3000/$6000 ON ded , 20/80 co-ins, $2500/$5000 OOP ($122.87/wk)

Anthem Std HDHP w/ HSA $3000/$6000 IN & $6000/$12000 ON, 30/70, $5000/$10000 OOP ($92.27/wk)

Anthem EPO $2000/$4000, $25/$75 copays, $5000/$10000 OOP ($144.06/wk)

Anthem PCP PPO $2000/$6000 IN & $4000/$12000 ON, 50%, $30/50% copay, ($88.27/wk)
